Subscription Plans

• Individuals:  A single adult can subscribe for $99/month.  This includes:

  • Access to the physician phone number during normal daytime hours (phone is set to "Do Not Disturb" from 9pm-7am daily), as well as through text and email.  Appointments can be requested by any of these means, and are available to be scheduled between 9am to 5pm weekdays.  

    • For the best service, priorities should generally be the following, :

      • Phone calls for urgent matters or those that would likely need questions answered etc.

      • Texts for shorter matters that are not as urgent, and could wait a little while if there is a potential that it could interrupt another patient's appointment

      • Emails for less urgent matters that would take more time to explain, are easier to be typed out, or need attachments etc. 

    • For the time being, permitting to see patients in the home office are pending with the city, so all appointments will be virtual.  If you need refills on controlled substances, I can do this for patients that are already under my care from the former practice, and I will hold off on further drug testing until patients can be seen in person.  If you are a new patient signing up and wish to have refills for controlled substances, I will need to send an order to the lab of your choosing to have a urine drug screen done prior to sending anything in.  

  • Yearly preventative exam.  For details of what is included with the annual exam, click on "Services" above. 

  • Lab testing:  This is done through a custom lab, MedScan, and there will be no additional out-of-pocket costs for all necessary labs.

  • Imaging:  At this time, no special arrangements have been made for imaging.  Orders will be placed as needed, and the imaging center bills you directly.  If your insurance company has a co-pay for this, you will be responsible for it.

  • Referrals:  All referrals needed will be placed in a timely manner.  

• Couples:  Adults who live together in any kind of relationship can sign up as a couple for $179/month, with the same access as individuals.

• Add-on: Couples or individuals who are guardians, who wish to add a child or dependent, can do so for $39/month

All of the above can be billed monthly, or every 3 months to a year.  

The electronic medical record only allows for pricing by age tiers, so after the first family member signs up, it will be adjusted manually.

One Time Fees

• Annual Exam Only:  Patients who need only an annual exam once per year can do this for a one-time charge of $149.  For details of what is included with the annual exam, click on "Services" above. 

• One-time sick or single-issue visit:  Patients who are not enrolled and wish to be seen for a one-time visit for a specific reason can do so, pending availability, but must make an appointment.  No walk-in appointments can be seen at this time.  The price for the appointment is $179, and includes the services needed for the appointment, along with any follow-up tasks specifically related to it (e.g. calls for test results, referrals for specialists etc.).  

• House Calls:  Depending on location, timing, availability, and necessity - This will be offered on rare occasions on a case-by-case basis.  The fee for this has not yet been decided.
